Causes of a perforated eardrum

There are many causes of eardrum perforation, including:

  • Scuba diving.
  • A sudden loud noise such as an explosion.
  • A hard blow to the ear.
  • Pushing a sharp object deep into the ear, such as cotton swabs.
  • Rapid changes in air pressure (Barotrauma),such as when flying.

Indications for tympanoplasty

The eardrum is a delicate membrane that is susceptible to perforation due to its fragility. Symptoms associated with perforation include ear pain, ear discharge and sometimes bleeding. Reduced hearing is also often observed.

Tympanoplasty may be indicated in several cases, including:

  • Chronic tympanic perforation that does not heal spontaneously and persists for more than 3 months.
  • Chronic otitis: inflammation of the middle ear lasting more than 3 months.
  • Otosclerosis: an abnormality of the middle ear that leads to conductive deafness.
  • Cholesteatoma: a middle ear cyst that can destroy middle ear structures.

Tympanoplasty process in Türkiye

During a tympanoplasty, an otolaryngologist usually uses a microscope to carefully examine the anatomy of the eardrum and middle ear. The surgeon may also use swabs to check for infection or pus. Depending on the size and location of the perforation, the surgeon may make an incision behind the ear or in the ear canal to access the eardrum.

Once access is gained to the middle ear, the surgeon will clean and remove any abnormal tissue. The damaged eardrum is then reconstructed with an autograft. The transplanted tissue is taken primarily from the aponeurosis of the temporal muscle behind the ear, the covering of the ear cartilage (known as the perichondrium),or the cartilage itself.

The removed natural tissue is positioned at the level of the tympanic perforation. The remaining layer grows through capillary action and closes the perforation. The choice of graft material depends on the size of the hole. 
Sutures are generally not required, although an absorbable sponge may be used to support the graft if necessary.

Postoperative recovery and care

After tympanoplasty, follow-up appointments are scheduled to monitor healing and detect any complications.

During recovery, patients may experience temporary discomfort, swelling, or discharge from the ear. Antibiotics may be prescribed to prevent infection.

Patients should carefully follow the post-operative instructions given by the otolaryngologist. These instructions may include

  • Keep the ear dry.
  • Avoid loud noises and strenuous activities.
  • Take prescribed medications as directed.

Most perforated eardrums heal within a few weeks or months after tympanoplasty, resulting in a significant improvement in symptoms. In some cases, however, a second surgery may be needed to achieve optimal results.
