What is orthotopic liver transplantation?

The surgery is very complicated, requiring careful crop of the donated liver and meticulous placement into the recipient.

The donor liver is connected by anastomosis at the vascular sites, with care taken to conserve hepatic blood vessels. The gall duct is usually linked to the recipient's. However, choledochojejunostomy (connection of the common bile duct to the jejunum) may be performed.

What is domino liver transplant?

DLT or domino liver transplantation is transferring a liver from a patient presenting a metabolic disease. The reciever is a patient with an end-stage hepatic disease.

This aims to reduce the effect of the recurrent syndrome in the recipient. The recipient will not develop the metabolic syndrome.

In a domino hepatic grafting, you receive the organ from a donor who has amyloidosis.

How is the intervention done in Turkey ?

The patient must be well-prepared for the hepatic grafting. A preoperative assessment is requested by the surgeon. A general pre-anesthesia consultation is mandatory.

The donor and the recipient must be relatives and have the same blood type. An immunological assessment of the donor and the recipient is requested. Several additional examinations must be carried out between a blood and radiological report.

The cross-match between the receiver and the donor must be negative (this is the rejection test). To prevent rejection, two to three immunity inhibitors are associated. Corticosteroids, azathioprine, ciclosporins and mycophenolate are indicated, three of them are selected.

The grafting is performed in an operating room while respecting the rules of strict aseptic to prevent any risk of infection.
