What is ICSI in IVF Treatment?
ICSI in IVF treatment is generally recommended when the male partner's sperm count or quality is limited. It is also used when other infertility factors prevent natural fertilization. This procedure, considered a major advance in the reproductive medicine field, has significantly improved the success rates of conventional IVF. By overcoming fertilization problems, this technique has become a valuable option for many couples struggling with infertility. However, it is important to discuss in detail with a MAR specialist whether intracytoplasmic sperm injection (ICSI in IVF) is the best option in a particular situation.
Principle of Fertility Treatment
Intracytoplasmic sperm injection (ICSI) is the best option for promoting fertilization by injecting a well-selected sperm into an egg. The procedure, performed at our partner centers in Istanbul, Turkey, is based on the injection of a single sperm. It is delivered directly to the target site, eliminating the risk of failed penetration. This is the difference from conventional IVF, where fertilization occurs naturally.
The process of forced sperm injection
The ICSI in the IVF process involves several steps, starting with the ovaries' stimulation and ending with fertilization.
Ovarian stimulation
In this phase, the woman must take medication to increase the production of mature oocytes for forced fertilization. In the second phase, the doctor aspirates the retrieved oocytes using a follicular puncture.
Sperm selection
The partner's spermatozoa are then prepared and selected according to motility, morphology of the head and flagellum, and several other factors that promote assisted fertilization. Selection of these male gametes maximizes the success of in vitro fertilization. As it increases the chance of obtaining a quality embryo.
Oocyte preparation
Before injecting the selected spermatozoon, it is necessary to remove the cumulus cells (decumulation) surrounding the ovum, more specifically the crown. This technique is based on enzymatic digestion to facilitate the spermatozoon passage to the genetic material of the mature egg with a micro-needle.
Injection technique
During this phase, the biologist initiates intracytoplasmic in vitro fertilization by injecting the sperm directly into the egg. In this process, two micropipettes are used, one to aspirate and fix the egg, the other to immobilize the sperm and carry out the injection with great precision.
Results of ICSI in IVF
Once implantation is complete, the Medical assisted reproduction (MAR) practitioner will cultivate the retrieved fertilized eggs (zygotes) in the laboratory until they become embryos. After the endometrium has been prepared, the embryos are transferred to the uterus for further development, where they are monitored. Subsequently, the woman undergoes tests to determine whether the pregnancy has been successful.
Indications for intracytoplasmic sperm injection
Generally, this micromanipulation technique is applied to treat certain fertilization and infertility problems. These include the following conditions:
- Severe male infertility: abnormal or low sperm count
- Obstruction of the male vas deferens: This prevents sperm from passing through.
- Oocyte abnormalities: Where the zona pellucida is very thick, sometimes requiring assisted hatching.
- Age: the older the woman, the lower the chance of becoming pregnant.
