    Coronary Angioplasty: What is it?

    Coronary angioplasty (transluminal dilatation or percutaneous transluminal angioplasty PTCA) is a cardiac procedure used to treat narrowing and blockage of the coronary arteries of the heart (atherosclerosis) caused by atherosclerotic plaque.

    Coronary angioplasty is a cardiac catheterization technique. It involves the insertion of a special catheter into the coronary arteries and the examination of the inside of the arteries through an X-ray machine and a contrast medium visible on X-rays.

    The main indications for the procedure are the presence of coronary artery obstructions that reduce the blood supply to the heart muscle, and myocardial infarction caused by acute occlusion due to arteriosclerosis.

    Indications for the procedure

    The coronary arteries are responsible for carrying oxygenated blood to the myocardium (heart muscle) to keep it alive.

    However, these arteries are the main targets of atherosclerosis. This condition is usually associated with aging and an unhealthy lifestyle, not to mention other factors such as family history.

    If too many fat layers and plaques build up inside them, then they may become blocked. This heart disease is known as atherosclerosis.

    Coronary artery disease, which occurs as a result of narrowing and blockage of the arteries, renders the heart muscle unable to receive oxygen-rich blood. In severe cases, this can lead to necrosis of a greater or lesser amount of heart muscle tissue (myocardial infarction).

    A coronary angioplasty procedure is a surgery done to widen clogged heart arteries.

    Pre-operational Preparation

    Before doing the operation, the doctor requires you to take some tests and follow certain instructions. For example, he may advise not to eat 8 hours before the operation.

    The medical assistants do all the necessary preparations needed for a successful operation. They make sure that all the surgical tools are sterile before the operation and that all medical instruments are present in the operation room.

    The operation 

    The procedure usually takes 2 hours under local anesthesia (the patient is awake during the operation but feels nothing). A catheter (a thin tube) with an inflatable balloon is introduced into the diseased artery at the level of the narrowed zone.

    Once the tube is placed, the balloon inflates and adopts the shape of the coronary artery to widen the diseased segment. After that, it will be deflated again to reopen the bloodstream and restore the flow.

    A coronary stent is a small tube with a net-like structure and a balloon inside it. It is used to release blood flow inside a blocked artery.

    The most common type of stent is called a drug-eluting stent (DES). This type of stent releases a drug at fixed time intervals. This helps stop the formation of cells that lead to vascular fibrosis (scarring of healthy tissue inside the vessels) and blood clotting. 

    After a stent insertion, the normal functions of the cardiovascular system are restored and the patient starts to feel better. Good results can be achieved after stent placement.

    Post-operation

    After the operation, the patient receives after-operative care in a specially equipped room, called a cardiac care unit (CCU). During this stage, his health status is monitored by the treating doctor.

    The patient can leave the hospital the next day.

    Risks and Complications of a PTCA Procedure

    Though coronary angioplasty procedure is a surgical procedure in the heart, it is much simpler than the Aoro-Coronary Bypass (PAC),sometimes called coronary artery bypass grafting. Some risks associated with a PTCA procedure are

    • Postoperative Fever: A surgery fever usually occurs in the following days after the operation.
    • Re-narrowing of the artery (restenosis): If the artery narrows once more over time, a new coronary bypass operation or angioplasty will be required.
    • Blood clots: Aspirin is combined with other medications during surgery to help diminish the risk of blood clotting inside the arteries.
    • Infection, hemorrhage, or artery lesions due to the insertion of the stent.
    • Bruises and sometimes bleeding in the arms or legs where the catheter was inserted.
    • Damage to the coronary artery: The internal walls may become torn.
    • Heart attack.
    • Stroke that can occur during the surgery if plaques break loose inside the coronary aorta. A stroke is an extremely rare complication because blood thinners are used combined with other drugs.
